Separation History and Physical Exams: All active duty members retiring or separating will need to request a Separation History and Physical Examination. The process should be initiated 180 days prior to your date of separation. This can be requested through the My IMR / ASIMS – Medical Readiness tab on the Air Force Portal.

AF Form 422 / Medical Clearance Reviews for overseas clearance, job retraining, job reclassification, special duty, security clearance, Palace Chase, Palace Front, developmental special duty, commissioning and PME attendance. Mental Health Assessment: After completing your Preventative Health Assessment questionnaire online, located in the My IMR / ASIMS tab on the Air Force Portal, please call the appointment line.
